Personal Information:  If any of your personal information such as name, address, phone number or insurance changes, please notify our office as soon as possible so we have a way to contact you if necessary.

Insurance Cards: Bring your insurance card with you to the office so we may copy it for our records. Copays will be due at the time of your visit. We accept cash, checks and credit cards (MC,Visa, Discover & AMX)

Referrals:  Because we are are speciality providers, many insurance companies will require a referral from your primary care physician before you can see our physicians. It is your responsibility to know if you need a referral and to obtain one from your primary care clinic before your visit.  This will prevent the insurance company from denial of payment when the claim is received.

Records & Xrays: If you have any medical records or xray films regarding your condition please bring them with you.

Release of Information from our clinic:  If you need to have records from our clinic sent to another facility you will need to sign a release of information form. For your convenience you may print Release of Records Form # 1 located in the Patient Forms section of our web site. Fax or mail completed form to our office.

For your convenience you may print Release of Records Form # 2 located in Patient Forms section of our web site. Fax or mail completed form to the appropiate clinic or to our office and we will forward it.
